The client’s situation, wishes and possibilities and impossibilities form the basis of the plan of

action. The personal assistant collects all of the information required and specifies the client’s

requests for help. She specifies the objectives for the supervision and activities in consultation

with the client. She formulates the plan of action and presents it to those involved for approval.

Based on the plan of action, the personal assistant formulates the activities plan in conjunction

with the client. She consults colleagues and experts as required about the specific crystallisation

for the client. She subsequently works up the activities plan with evaluation moments and criteria.

The personal assistant supports the client in taking ownership of his own personal life. If the client

requires support in taking ownership of his personal life, he is nevertheless the starting point in

any decision that concerns him. The personal assistant asks the client to exert influence on the

support provided where, when and by whom.

The social system may play a role in supporting the client. It is the personal assistant’s task to

reinforce the relationship between the client and the social system. The personal assistant carries

out activities to enhance the coping ability of the social system. She equips the social system with

information and involves it in the support for the client.

in life, are informed of his coping ability and know what he can realise in conjunction with his

Demonstrate that you provide and organise the support the client requires to take (greater)

ownership:

- make agreements with the client about what he can do and where caregivers and/or

experts are required

- inform the client about the support and the agreements made
