The personal assistant in disabled care participates in relevant consultations with colleagues for
the coordination of activities regarding care and support. She transfers activities and makes
agreements about the care and support and any bottlenecks concerning this.
The personal assistant in disabled care evaluates the support provided on a regular basis. She
collects various data and analyses them. She writes the evaluation report or evaluates verbally with
her manager. She discusses the information from the evaluation with the client and the team.
The personal assistant also evaluates the execution of the coordinating tasks and adjusts this as
required.

customary in your institution. Present written supporting documents of the transfer of your
activities.
A smoothly running support in which quality, continuity and consistency are safeguarded.
Submit the following supporting documents:
- A fully completed assessment form substantiated by the examiner
- Written supporting document of transfer
