Work process 1.9 Evaluates health care delivery

Assignment 1 Evaluate the overall nursing care of patients. Evaluate with others involved. This evaluation may be a periodical evaluation, for instance as the preparation for the MDC or a final evaluation because the care has been terminated or the patient has been transferred. Assignment 2 Submit an accountability report in which you account for the choices you have made regarding the evaluation. Submit a professional product that has been signed by your supervisor to support your report. Choose from the following options:  an evaluation report for the benefit of the MDC which includes the propositions to adapt the nursing care plan

 an adapted nursing care plan, clearly stating the adaptations carried out

 a transfer with propositions for further nursing.

Result You have evaluated the health care delivery regularly and carefully and adapted the nursing care plan as required. You have formulated a (final) evaluation of the entire nursing process.

Supporting documents Provide the following supporting documents:

 A fully completed assessment form substantiated by the examiner

 A report about the accountability of the choices made within the nursing process
