Your exam is taken in a moderately complex care situation.
You independently support the patient in personal care, such as eating and drinking, using the
toilet and mobility. You adapt to the capabilities and experiences of the patient. You observe and

Evaluate the support of the entire care. Evaluate with others involved. This evaluation may be a
periodical evaluation, for instance as the preparation for the Multi - Disciplinary Consultation (MDM)
or a final evaluation because the care has been terminated or the patient has been transferred .
Submit an accountability report in which you account for the choices you have made
regarding the evaluation. Submit a professional product that has been signed by your supervisor
to support your report. Choose from the following options:
an evaluation report for the benefit of the MDM with the propositions to adapt the care plan
an adapted care plan, clearly stating the adaptations carried out a transfer with propositions for further care

This exam is taken in a moderately complex care situation.
The nurse in individual healthcare carries out nursing technical procedures in a competent and
professional manner in accordance with the applicable protocols, laws and regulations and the
guidelines of the organisation.
The nursing technical procedures also include reserved and high-risk procedures.
In addition, the nurse in individual healthcare in maternity care provides assistance in childbirth.
She assists the woman giving birth from the start of childbirth. She assists the midwife or
obstetrician during childbirth.
A woman giving birth is assisted and means and resources are deployed efficiently in the execution
of the nursing technical procedures and parturition assistance.
The carer provides clear and understandable information, advice and instructions in the execution
of her tasks to the woman/women giving birth, midwife, next of kin and/or caregiver.

You carry out the nursing technical procedures and reserved nursing technical procedures (see
appendix) according to protocol and existing legislation and you assist the woman giving birth as
part of this.
Make sure every one of your procedures are evaluated. Use a training checklist to keep everything
updated, the procedures you master and in which you are thus competent.
If you can prove that (certain) nursing technical skills are not or rarely executed in practical
vocational training, execute these in a simulated setting.
Have yourself evaluated using a random check to assess whether you have mastered the work
process. The following condition applies: you must demonstrate using the checklist that you are
competent. Make agreements with your assessor.
Write a report about the nursing technical and reserved procedure(s) you have carried out for a
patient.
This report elaborates:

Before having this work process evaluated, you must demonstrate that you are competent to carry
out parturition assistance.

The carer monitors, supervises and encourages the client in maintaining psychosocial and
social self-help skills. You encourage the client to direct his/her life and to maintain his/her
identity and fulfilment of life as much as possible. You also counsel the client in dealing with
and handling the consequences brought about by the handicap, disability, therapy or
treatment.
Provide guidance in psychosocial and social function, in maintaining and reinforcing the
social network and entering into and maintaining contacts.
Create the most optimal living environment possible for a group and include the next of kin
and/or volunteers as required.
Promote the most optimal group climate possible. Observe the group processes and
behaviour of the clients. Help the client to obtain insight into his/her behaviour and the
effect of this behaviour on others in the group. Mediate or intervene in the case of conflicts.

Explore the issues of this client using theory. Involve:
the nature and causes of this client’s disability the consequences of this disorder or disability for the way of living and the experience of the client in maintaining autonomy

Write a supervision plan in which you explain the following topics:
